1. Proof of Identity

a. First, middle, and last name b. Any previously used names c. Address d. Social Security Number e. Date of Birth f. Copy of a government issued ID


2. TraAicking Documentation

a. Provide victim determination documentation received from one or more of the following:

• A Federal, State, or Tribal entity.

• A non-governmental organization or human traAicking task force including victim service providers aAiliated with these entities, authorized by a Federal, State, or Tribal government entity

• A self-attestation that is signed or certified by a Federal, State, or Tribal government entity, court of competent jurisdiction, or an authorized representative of these entities.



3. List of Adverse Items of Information.

a. Provide a document that lists items of adverse information that you believe should not be included by a consumer reporting agency because the items results from a severe form of traAicking in persons or sex traAicking.


4. Your preferred method of written communication. This can be a mailing address or email address. All communication and final determination will be provided to you through your preferred method of communication.
